- How have you confirmed that you are pregnant?
- Have you confirmed that you have a viable, uterine pregnancy? (Viable means one that is capable of developing under normal conditions.)
- Are you aware of the emotional consequences that may follow an abortion?
- Are you familiar with the physical risks that may follow an abortion?
- What are your legal rights before and after an abortion?
- What do you know about abortion procedures?
- Which abortion procedure will you be having?
- What do you know about fetal development?
- Do you know what other procedures you will have to undergo at the abortion clinic? What about any procedures after the abortion.
- Are there are any costs over and above the cost of the abortion procedure?
- Do you have a sexually transmitted disease?
- Has the abortion facility informed you where you will be transported by ambulance if you are injured during the procedure?
- You have a choice. Has your abortion provider explained other alternatives?
